The Physical Therapist will plan and administer personalized rehabilitation programs for patients within their homes to improve strength and mobility. They are also responsible for documenting patient progress and coordinating care with the clinical team and physicians.
Candidates must hold a current and unrestricted Physical Therapy license and valid CPR certification. Experience collaborating with a multidisciplinary healthcare team is required, with one year of professional experience preferred.
