The Case Worker assists patients in keeping appointments, identifies barriers to care, and coordinates the development and implementation of treatment plans. They also conduct patient evaluations, provide medication adherence education, and facilitate linkages to community resources and research staff.
A Bachelor's degree in a relevant field such as psychology, social work, or child development is required. Candidates must have at least one year of relevant experience working with children, families, or individuals with special needs.
